Army ROTC Cadets will fill out the CC Form 104-R. Once completed for all semesters required of the academic program, you will submit that form electronically to your academic advisor. For specific instructions, please follow the instructions provided in the document provided below.
For all cadets
Keep in mind that completing the academic planning form does NOT constitute academic advising for the semester. There are a number of topics advisors will cover in the semester advising appointments that are not approached in a review of your form.
Also, PLAN AHEAD! It will take time to complete the review of this form. To ensure that the form contains all the requirements to ensure your timely graduation and commissioning, advisors will need at least two business days to look at the form. If there are edits needed, it may take longer. So, plan ahead!
