New Faculty Employment Forms

New Baylor Faculty can access employment forms online in IGNITE. Ignite is our new operating system used for Human Resources, Budget and Payroll.

You will review and complete the forms listed below, prior to or no later than your first day of employment. If you have questions about any of the employment forms, please email askHR@baylor.edu.

Forms to Complete

Form I-9
  • Complete section 1 of this form online, on or before your first day of work. Form I-9s are being completed virtually, please email askHR@baylor.edu to make an appointment.
  • Bring the required original, unexpired documents - either one item from list A or one item each from lists B AND C - with you to your virtual I-9 appointment in order to complete section 2.
    • Failure to provide acceptable documents will affect your employment status.

Add or Update your Direct Deposit Information
Electronic W-2 Form Election

Complete this form online and print a copy for your signature to authorize receipt of an electronic version of your year-end Form W-2 rather than a paper copy.

Faculty Pay Option

Faculty have the option to receive their ten-month academic year salary in twelve installments. This form allows you to elect into this pay option. Read the form for more details.

Self Identification Disclosure

Read the form, check boxes as appropriate, and then complete it online with your e-signature in Ignite.

Payroll Deduction Authorization

Read this form then print for your initials and signature.

Memorandum of Understanding for Faculty

Please review each item listed on this form make sure you understand what is required of you. There is a copy for your records and a copy to submit with your paperwork packet.

Copies of Unofficial Transcripts

Unofficial transcripts are required for all positions requiring a degree, with the exception of Baylor transcripts or transcripts previously submitted to our office.
