NOTE: On the last page of this form prior to submitting, please upload the following documents in a single Word (.docx) or PDF (.pdf) document:

• A copy of written approval from Student Activities (if applicable)
• Proof of liability insurance for your national organization (if applicable)
• A detailed diagram of your event layout if it includes a major production (concerts), stages, inflatables, grills, tents, or any type of open flame (if applicable)
