Process for Travel With Students
1. Fill out both the Domestic Travel Form and Participant List and submit it to the Insurance Program Manager 10 days before departure. On the Domestic Travel Form include names of all trip planners. Fax to 710-6256
2. Following the submittal of the two forms mentioned above, you (Faculty Advisor and Contact Person) MAY be contacted by e-mail from the Office of General Counsel IF your travelers are required to sign release forms prior to traveling. Daily check your email, and follow all guidelines provided by the Office of General Counsel.Contact for questions:
