The Proposal Routing Forms (PRFs) below are intended for use by faculty, department chairs, and deans as they propose new programs and degrees or make revisions to those currently in place.All files are in writable PDF format. In order to save changes, be sure to use Adobe Acrobat 9.0 (not just Adobe Acrobat Reader). The forms also should open and save using Macintosh's Preview program.
Even though forms may be produced online, they must be printed and signed as they move through the approval process.
