In order for the camper to be eligible to participate, the applicable forms must be completed and a HARDCOPY mailed to the Baylor Tennis Office ASAP but no later than 10 DAYS before camp starts. (Mailing address is – 1500 S. University Parks Drive, Waco, TX 76706). If you complete the online registration these forms will be emailed to you but you must still mail them to us.

PLEASE NOTE:::::

Every adult participant or parent/legal guardian of under age participants will need to fill out in Blue or Black INK the release forms and the medical information form. (A separate form MUST be filled out for each person even if they are a part of the same family).

All individual participants age 18 or older will need to sign a release (FORM E) and the parents or legal guardians of all individual participants under the age of 18 will need to sign a release (FORM D).

***IF ANY FORM HAS BEEN ALTERED, MARKED WITH REVISIONS IN ANY WAY, OR HAS NOT BEEN PROPERLY SIGNED, THE PARTICIPANT WILL NOT BE ABLE TO PARTICIPATE OR REGISTER ON CAMPUS UNLESS ANOTHER FORM IS COMPLETED CORRECTLY AND WITHOUT ALTERATIONS, which means minors MUST have their parent of legal guardian present.

Only original forms will be accepted, scanned documents, faxes, photocopies, etc. will not be accepted as complete
