We are trying to find really great people and players to staff/counselor our summer programs. If you know of any one looking for a unique summer employment experience we hope you'll let them know about our summer tennis camp at Baylor

Before completing this application please remember a complete background check is performed on all applicants.

All counselors/staff are required to complete either applicable Form D or E and the Medical Information and Release -

IF ANY FORM HAS BEEN ALTERED, MARKED WITH REVISIONS IN ANY WAY, OR HAS NOT BEEN PROPERLY SIGNED, THE PARTICIPANT WILL NOT BE ABLE TO PARTICIPATE OR REGISTER ON CAMPUS UNLESS ANOTHER FORM IS COMPLETED CORRECTLY AND WITHOUT ALTERATIONS, which means minors MUST have their parent of legal guardian present.

Only original forms will be accepted, scanned documents, faxes, photocopies, etc. will not be accepted as complete.
