ONLINE ROSTER
Use the Online Roster to track your membership, check on meeting locations and times, submit service hours, check the student organization leadership directory, and more! STUDENT EVENTS CALENDAR
Check the calendar for your organization's events, meetings, or activities...or for other organizations! Utilize this resource when planning your event to ensure that your event does not conflict with others!

ONLINE EVENT REGISTRATION

Register your organization's events and activities online. It's a quick and easy process that is necessary for approval. Please read the Online Event Registration Instructions.

T-SHIRT REQUEST FORM INSTRUCTIONS
Student organizations may use the online t-shirt request form for t-shirt and other product approvals. T-shirt designs are required be submitted to the Department of Student Activities for approval PRIOR TO PRINTING. T-shirts using the Baylor name, trademark or University graphics must also be approved by the Office for Collegiate Licensing and may require additional time for the approval process.

TRAVEL AND DRIVER FORMS
In order to travel using personal vehicles for your organization, you must submit the Approved Driver Form and the Domestic Travel Form and Participant List to the Department of Risk Management.

SOLICITATION REQUEST FORM
Organizations wishing to sell items on campus (as fundraisers or otherwise) are required to submit the appropriate form to the Department of Student Activities for approval by the Sales & Solicitation Committee.

SPECIAL EVENT REQUEST FORM
Organizations hosting an event that qualifies as a "Special Event" will be required to submit the Special Event Request Form. For more information, please visit the Risk Management website. Please note that Special Event Request Forms must be received no less than one month (30 days) prior to the event for consideration.

NEW MEMBER EDUCATION
Organizations participating in a new member education process should register this process by submitting the appropriate documents to the Department of Student Activities no less than four weeks prior to implementation. No student organization shall make any requirement of any new member which is not listed in the submitted program.

ORGANIZATION E-MAIL REQUEST
Request a Baylor University organizational e-mail address (for example: ORGANIZATION_NAME@baylor.edu).

SPEAKER APPROVAL FORM
Organizations wishing to bring a speaker to campus for an event or activity are required to have their prospective speaker approved.

WAIVER FORM
Organizations in need of waiver forms for specific events or activities may use this sample waiver form to help protect the organization and its members in the event of an accident. Please note that this form may not cover all risks for all events or activities. Please read it carefully to ensure it meets your organization's needs. It is recommended that you consult a licensed attorney to ensure the organization's protection.
