Field Application Process

Field Application Processes

MSW Foundation (Generalist Practice)

In order to apply for Field, a student must meet the following eligibility requirements:

  • Completed or enrolled in first semester Social Work courses: SWO 5221, 5362, and 5561
  • Minimum of “B” in Practice courses
  • Attend Field orientation before entering Field (1 session is offered the week before classes begin in August; 2nd day of MSW Foundation orientation)
  • Minimum of "C" in the Introduction to the Profession Course  (5221/5121)

Once the student has been accepted in to the program and has sent in the statement of intent, the Field office will send an email with the application materials. These include:

  • Field Registration/Application (turn in 3 copies)
  • Disclosure Policy and Information Sharing Form (turn in 2 copies)
  • Transcript Permission Form (turn in 1 copy)
  • Student Code of Conduct (turn in 1 copy)
  • Internship Readiness Form (turn in 1 copy)
  • Reference Form from 1 reference: Volunteer/Employment Supervisor
  • Students also turn in 3 copies of current resume

These completed materials are submitted to Krista Barrett, Program Manager, Field Education. Once submitted, Melody Zuniga, Director of Field Education-Generalist Practice, and Rob Rogers, Associate Dean of Graduate Studies, will review and recommend that the student is admitted to Field. The student interviews with Melody Zuniga, who will work with the student on placement decisions. The interview may take place either in person or over the phone.

MSW Concentration (Advanced Practice)

To enter the Concentration year placement, the MSW Foundation student must have:

  • Attended Field orientation offered the first day of the spring semester
  • Successfully completed or be concurrently enrolled in all required SWO graduate course SWO 5221/5121, 5362, 5561, 5381, 5463, 5322, 5491, and 5492

There will be additional paperwork to complete during the spring semester before the Concentration year begins. This paperwork includes:

  • Field Registration Addendum (updates on Field Application/Registration; turn in 2 copies)
  • Disclosure Policy and Information Sharing Form (turn in 2 copies)
  • Transcript Permission Form (turn in 1 copy)
  • Internship Readiness Form (turn in 1 copy)
  • Internship Interest/Focus Sheet (turn in 1 copy)
  • Current updated resume (turn in 2 copies)

These completed materials are submitted to Krista Barrett. The student then interviews with Erma Ballenger, Director of Field Education-Advanced Practice, concerning the Concentration year internship.

Advanced Standing (Advanced Practice)

In order to apply for Field, a student must meet the following eligibility requirements:

  • Accepted in to the MSW program
  • Must attend Field Orientation (offered in the summer; 2nd day of Advanced Standing orientation)

Once student has been accepted in to the program and has sent in the statement of intent, the Field office will send an email with the application materials. These include:

  • Field Application/Registration (turn in 2 copies)
  • Disclosure Policy and Information Sharing (turn in 2 copies)
  • Transcript Permission Form (turn in 1 copy)
  • Student Code of Conduct (turn in 1 copy)
  • Internship Readiness Form (turn in 1 copy)
  • Students also turn in 2 copies of current resume

These completed materials are submitted to Krista Barrett, Program Manager, Field Education, either in person or through the mail. The student then interviews with Erma Ballenger, Director of Field Education-Advanced Practice, who will work with the student on placement decisions.
