The newly revised LHSON Health Form is required of all students entering the Nursing School in Dallas. This form will be made available during Online Orientation. Included on this form are required immunizations, as well as a physical examination. All Students MUST have a Physical!! Immunizations can be obtained from your choice of location/physician. Once you have a BUMC ID, you qualify to use the BUMC Employe Health Center listed below.
BUMC EMPLOYEE HEALTH CENTER:
Students are allowed to go to the Employee Health Center at Baylor University Medical Center for illness, shots and some tests. You will need to pick up a yellow form filled out by one of the Administrative staff prior to your visit and must return the signed slip to Student Services when finished. A doctor is only at the clinic from 8:00am to 9:30am Monday thru Friday. There is no fee to be seen by the doctor, however there is a fee for shots and tests. A cost list will be in your packet during Orientation, or you may call Employee Health at 214-820-3323.
