Protocols/Tools Protocols need to be established regarding team leadership and membership, the types of deaths to review, the purpose of the review, the review process, confidentiality and sharing information, producing reports, disseminating information, and other key issues. Each team will develop its own set of protocols about how to conduct reviews. New teams need not reinvent the wheel. Existing teams have developed protocols that can help serve as a guide to new teams. Please note, while it is helpful to see and draw upon the work of established teams, it is important to go through the process of formulating protocols as part of team building.
