Please make sure that your computer has Adobe Acrobat Reader 6.0 or greater.

Adobe Acrobat Standard or Professional is required to save the electronic form. A trial copy can be downloaded at:

Please fill out the application and submit it using one of the following methods:

1. Email - Download the application and save it to an easy to remember location (ex: the desktop). Fill it out, save it again, attach the completed form and submit to Delores_Oliver@baylor.edu.
2. Drop it off - Deliver the application to the Administrative Manager's office, Moody Library G19, on the Garden Level of Moody Library.
3. Mail it in - our mailing address is:
