Baylor University employees who log in to Blue Access for Members between October 1, 2012, and October 31, 2012 and complete a Health Assessment are also entered into a second drawing for a chance to win an Amazon Kindle Fire.

*One Amazon gift card will be given away. Employees who have completed the assessment since January 1, 2012, will also be eligible for the Kindle incentive. One Kindle Fire will be given away. Participants must be 18 or over to win. All entrants must be a legal U.S. resident. Your odds of winning will depend on the total number of entries received, with one entry per person.
