Please review, complete and print the forms in the New Faculty Packet to the right on this page and bring them with you to your Faculty First Forum. This will greatly expedite the employment and benefit enrollment process.

Important notes about completing paperwork on-line:

  • Most of the forms in the packet are "fill-able" online, but not "save-able." If you close the window that contains the form, any information you have typed will be lost. Be sure to print your completed forms or save them to your computer before closing the window.
  • You can use the "tab" key to navigate between spaces on the forms.
  • If you have questions about any of the forms, you will have a chance to ask them during the Forum. If you prefer, you may wait to sign the printed forms until after your questions are answered.

Please review, complete and print the forms in the New Faculty Packet to the right on this page as instructed before coming to the Faculty First Forum. All forms listed below are included in the packet except the Form I-9, which must be completed online through the electronic I-9 system.

Personnel Information Form

This form gives Baylor the information used to make sure you are paid in a timely manner, and also to set up your access to the Baylor computer system.

Memorandum of Understanding

These are basic agreements you make with the University regarding the nature of your employment.

Form I-9

This document protects you from discrimination on the basis of national origin or citizenship status. It also allows Baylor to collect required proof of your employment eligibility. You must complete section one of this electronic form on or before your first day of employment; click here to begin, OR come to Human Resources to complete the form I-9. Section 2 must be completed in the presence of an HR Representative; don't forget to bring the appropriate forms of original, unexpired documentation with you! Electronic W-2 Authorization

This form allows you to receive your year-end W-2 electronically.

W-4

This form allows Baylor to withhold the correct federal income tax from your pay.

Conflict of Interest Policy and Statement

Please review this important university policy. All faculty and staff are required to complete the statement annually. You will be able to do so after you are assigned your BU ID number.

12-Month Pay Option

This form allows faculty members the option to receive their ten-month academic year salary in twelve installments. Click here for the monthly pay schedule.

Payroll Deduction Authorization

For your convenience, your Baylor ID card can be used to charge purchases for various university services (food, immunizations at the health center, etc.). This form gives us permission to deduct those charges from your pay. NOTE: Most campus dining facilities only accept the BU ID card; no cash or checks.

Request for Deposit of Payroll Earnings

The purpose of this form is to gather the information needed to deposit your paycheck directly into your bank account. Baylor uses direct deposit as its primary means of payment.

Self-identification Forms

Baylor University is subject to certain governmental recordkeeping and reporting requirements for the administration of civil rights laws and regulations. In order to comply with these laws, we invite employees to voluntarily self-identify their race, ethnicity, disability and veteran category.

Prior University Work Experience

To determine whether you are eligible for University contributions to your retirement account more quickly, provide details of previous work experience as faculty or staff at a college or university.
