Every individual who desires services from our facility must be evaluated first. The process is simple.
  • Call Mrs. Melody Moon, Clinical Administrative Associate, at 254-710-2568 to schedule an appointment.
  • Print and complete the consent and case history forms listed below. If unable to print, please let Mrs. Moon know.
