Diagnostic Evaluations

Diagnostic evaluations are conducted on a weekly basis during the fall and spring semesters. The purpose of the evaluation is to:
  • determine the problem
  • assess communicative competence in all parameters
  • determine prognosis
  • determine recommendations
Every individual who desires services from our facility must be evaluated first. The process is simple.
  • Call Mrs. Cheryl Wade, Clinical Administrative Associate, at 254-710-2568 to schedule an appointment.
  • Print and complete the consent and case history forms listed below. If unable to print, please let Mrs. Wade know.
