Please print and complete the form below and bring it to your first appointment. This is part of the paperwork that will need to be completed before the first appointment. If you do not wish to complete this form now, you may complete it at the Counseling Center prior to your first appointment. You will need to arrive, at least 30 minutes, before your appointment to complete all of your paperwork. If you choose to complete the online paperwork before your appointment, you will still need to arrive at least 20 minutes before your first appointment to complete the remaining paperwork.
