Open Enrollment

Open Enrollment for 2010 begins November 1, 2009 and ends November 30, 2009


Should you need help at anytime during the open enrollment process, please contact Compensation and Benefits, ext. 2218.

Open enrollment is a period of time each fall for you to make benefit changes for the following calendar year, effective January 1. Enrollment or cancellation of participation in group medical, dental, cancer, intensive care, recovery plus, or accident insurance products may only be done during the open enrollment period. Eligible faculty/staff who reject or drop coverage may not enroll until the next open enrollment period unless qualified based on a change in family status. All completed open enrollment forms should be mailed to: Compensation & Benefits, One Bear Place #97052, Waco, TX 76798-7052. PLEASE RETAIN A COPY OF ALL FORMS SUBMITTED FOR YOUR RECORDS.

2010 Medical and Dental Plan Changes


  • Flexible Spending Accounts
    Participation/continuation in flexible spending accounts requires annual enrollment.

    To request reimbursement from your flexible spending account

  • Salary Redirection Agreement (pdf)
    Required to enroll or re-enroll in the flexible spending account.

  • To add the group medical plan (BCBS/CVS CAREMARK):
  • Employee Premiums

  • Medical Enrollment Application/Change Form
    Life Insurance beneficiary would not be added to this form in Section 5. All other sections would be completed if applicable.
  • Salary Redirection Agreement
    Adding medical coverage will add a deduction to your payroll check.
  • To cancel the group medical plan (BCBS/CVS CAREMARK):

  • Medical Enrollment Application/Change Form
    Skip Sections 4 through 9. All other sections would be completed.
  • Add/Drop Insurance Product
    Note cancel BCBS medical plan on this form.
  • To add/cancel dependents to your group medical plan (BCBS/CVS CAREMARK):

  • Medical Enrollment Application/Change Form
    Life Insurance beneficiary would not be added to this form in Section 5. All other sections would be completed if applicable.

  • To add dependents to your DR dental plan:
  • DR Premiums/Plan
  • DR Dental Application/Change Form
    List names of dependents to be added.
  • Salary Redirection Agreement
    Adding dependents to DR dental will add a deduction to your payroll check.
  • To cancel dependents from your DR dental plan:

  • DR Dental Application/Change Form
    List names of dependents to be canceled.
  • To change your dental provider from QCD to DR dental plan:

  • DR Dental Plan
  • DR Dental Application/Change Form
  • Add/Drop Insurance Product
    Note cancel QCD dental and add DR dental.

  • Salary Redirection Agreement
    Adding dependents to DR dental will add a deduction to your payroll check.

  • To change your dental provider from DR dental to QCD dental plan:
  • QCD of America
  • Red or White Plan and Enrollment Forms
  • Add/Drop Insurance Product
    Note cancel DR dental and add QCD dental
  • 2010 QCD Premium Schedule
  • Salary Redirection Agreement
    Adding dependents to the White Plan will add a deduction to your payroll check.

  • To cancel Accidental Death & Dismemberment (AD&D), AFLAC products or Supplemental Term Life:
  • Add/Drop Insurance Product
    Note cancellation of the product you are canceling

    Applications are forwarded to Fort Dearborn Life's underwriting department for approval. Enrollment in the supplemental term life benefit can be requested during the calendar year.

    Baylor Retirement Income Plan
    Employees enrolled in the Baylor Retirement Income Plan are fully vested in Baylor sponsored retirement contributions, and may transfer these contributions between plan vendors at any time. Contact the Compensation & Benefits Office for more information.
