Parent PLUS Loan Release Authorization
Form to designate if Parent PLUS loan borrower wishes for student to receive PLUS loan credit.

Direct Deposit Authorization
Authorizes direct deposit for refunds of excess student financial aid and other credit balances. Must remit signed original with voided check.

Outside Scholarship Report Form
Informs the University of outside scholarships awarded.

Guaranteed Tuition Application
Application to enroll in the Guaranteed Tuition Option.

BearBucks™/Dining Dollars Request Form
Request for unused BearBucks™ and/or Dining Dollars when leaving Baylor permanently.
