Please print and complete the proper registration and waiver form above and turn it in at the SLC Service Desk to complete the registration process. Fees may apply.

Guests:
A current Baylor cardholder may bring up to 5 guests/day. Guests must present a picture ID, properly complete a guest waiver form and pay all applicable guest fees before entering.

Adding Family Members:
In order to add a dependent, student spouse or grandchild to a Campus Recreation Family Account, the Baylor ID holder and additional family member are asked come to the SLC Service Desk together to complete the process.
