Help Baylor Plan for Fall: Report Your Vaccination

Vaccination against COVID-19 is a safe and effective way to protect the Baylor community and provides a vital step toward a return to more normal campus operations in the fall. Members of the Baylor Family who have been fully vaccinated against COVID-19 are asked to share their vaccination status with the Baylor Health Center. By reporting your status, you help the University more accurately understand how well our community is protected.

Students, faculty and staff who have been fully vaccinated with a COVID-19 vaccine authorized by the FDA for emergency use are asked to fill out this simple Vaccination Reporting Form, which includes uploading a photo of your completed vaccination card. Vaccine information reported to the Health Center is protected health information covered by HIPAA and has the added benefit of providing you with exemption from weekly testing and quarantine.

“The importance of getting and reporting your COVID vaccine cannot be overstated. The University is working hard to provide a campus experience as close to normal as possible while remaining safe. Participation in vaccine efforts by receiving and reporting your vaccination helps us prepare for the fall semester as we think about campus activities and events,” Dr. Sharon Stern, medical director of Baylor Health Services, said.
