- Printable Baylor Application- Note: You must submit a $50 application fee with this application. The application fee will be waived if you apply online. For information about applying as a Graduate Student, see the Graduate School web site.
- Health Form (68KB PDF)- We recommend this form be submitted to Health Services on or before May 1 for Summer and Fall semesters and November 15 for the Spring semester.
Due to a new Texas law, all incoming students who reside on campus must have received immunization against bacterial meningitis at least 10 days prior to moving into any on-campus housing. In order to be considered for a first-round housing assignment at Baylor, we recommend you submit your health form with a documented meningitis vaccination (including the month, date, year and a health professional signature or stamp) by May 1.

Re-Activation Form - This form is used only in the following circumstances:
*If you wish to re-activate the Undergraduate Admission Application for a new term. (If you also wish to change from a non-degree seeking to degree-seeking student, you must complete the Full Undergraduate Application at baylor.edu/admissions/apply.)
*If you wish to re-activate your Baylor record because you have not attended Baylor since 1986. (If you have attended since 1986, your record is on file and you do not need to reapply.)
*If you wish to re-activate your Baylor record as a former provisional student so that you may return to Baylor.
