Information Required for any Enrollment Certification Request
Enrollment/Degree Certification Form
- Current name and any other name used while attending Baylor
- Baylor ID number
- Daytime phone number
- Semester(s) you wish to be verified
- Fax number, email address, or mailing address or addresses where enrollment certification is to be sent.
- Student's written signature (required only if non-directory information is being released)
- If enrollment certification is for deferment of loan(s), your account number(s) must be included.

Directory information includes current and past enrollment dates with number of enrolled hours, student status, expected date of graduation, and awarded degrees.
Non-directory information includes grade point average, social security number, and Baylor ID number. In order for non-directory information to be released, the student's signature is required.

Information Required for any Degree Verification Request
- Current name and any other name used while attending Baylor
- Graduation date and degree obtained
- Baylor ID number
- Daytime phone number
- Fax number, email address, or mailing address(es) where degree verification should be sent.

Forms such as deferment forms from lenders and good student discount forms can be mailed, e-mailed, or faxed. A good student discount form requires the written consent of the student (via the student's signature) before the Office of the Registrar can process it.
Once a proof of enrollment form has been completed by the Office of the Registrar, it can be mailed, e-mailed, or faxed.
