Completing the Signature Page (PDF format)
These files are PDF forms which will allow you to easily tab through the fields without disturbing the format of the form. Enter the appropriate data into the field, then tab to the next field. There are directions for using special formatting or characters if needed.
Field 1 Title Field: Enter the title of your Dissertation/Thesis. For most users, you will only need to enter the appropriate data into the field, then tab to the next field. However, if you need special formatting or characters in the Title Field (such as Italic), you must highlight the word that needs the formatting then RIGHT CLICK and choose Text Style. If you have special characters other than those on a traditional keyboard (such as Greek or advanced mathematical symbols), you must type the title in Word first. Copy it (ALL THE WAY TO THE paragraph marker), then Paste it into the PDF form field.
Field 2 Enter Your Name: Enter your name followed by any degrees you hold. List each degree, beginning with the first degree earned, using a comma and a space between your name and the degrees, and between each degree (i.e. John P. Doe, B.A., M.A.).
Field 3 Choose From List: From the drop menu choose either Dissertation or Thesis.
Field 4 Enter the Approved Department: You will type Approved by the Department of “department name” (i.e. Approved by the Department of Economics).
Field 5 Enter Department Chairperson’s name: i.e. John P. Doe, Ph.D., Chairperson.
Field 6 Enter Degree Name: Type the name of the degree you are seeking, (i.e. Doctor of Philosophy or Master of Arts).
Field 7 Approved by: Type Approved by followed by the appropriate committee type (i.e. Approved by the Dissertation Committee).
Field 8 Enter Name for your dissertation or thesis committee Chairperson: Again you will type the chairperson’s name, followed by the degree, then the word Chairperson (i.e. Jane A. Professor, Ph.D., Chairperson).
Field 9 Continue entering remainder of committee names: Type each name and degree separated by a comma and a space (i.e. John P. Doe, Ph.D.).
Final Field Enter Month and Year of Graduation: Type the Month and Year you plan to graduate (i.e. May 2013).
